E-Sign Consent Agreement – Care.com Money Prepaid Mastercard

Engage Financial Technology, Inc. (Engage) may provide you directly or indirectly through its third party service providers with certain communications, notices, agreements and policies that are required to use our Care.com Money Prepaid Mastercard app and related website (the “App”) and/or related services (“Services”) (including, but not limited to the Engage Privacy Policy and the Engage Terms of Use), transaction or account notifications, account statements, or disclosures in writing (“Communications”) regarding our App and/or Services. Your agreement to this E-Sign Consent confirms your ability and consent to receive Communications electronically with respect to all of the Services from Engage, and its third-party service providers, and to the use of electronic signatures in our relationship with you (“Consent”). You further agree that your computer or other device satisfies the system requirements set forth below and that you have provided us with a current email address or phone number at which we may send you Communications. If you choose not to agree to this Consent or you withdraw your consent, you may be restricted from using the Services.

Electronic Delivery of Communications and Use of Electronic Signatures

Under this Consent, Engage may provide all Communications electronically by email, by text message, via the designated Engage website caremoney.us or via the App.

Changes to System Requirements

If our system requirements change and that change would create a material risk that you would not be able to access or retain electronic Communications, we will give you notice of the revised system requirements. Continuing to use our Services after receiving notice of the change is reaffirmation of your consent.

Paper Delivery of Communications

You have the right to receive Communications in paper form. We have no obligation to, and will not, send any Communications in paper form unless requested by you to do so. To request a paper copy of any Communication at no charge, please write to Care.com Money Prepaid Mastercard, PO Box 421221, San Diego, CA 92142 or email support@caremoney.us, specifying in detail the Communication you would like to receive.

Withdrawal of Consent to Electronic Communications

You may withdraw your consent to receive electronic Communications at any time, by writing to the address set forth in the above preceding paragraph. However, withdrawal of your consent to receive electronic Communications may result in termination of your access to Services. Any withdrawal of your consent will be effective after a reasonable period of time for processing your request.

Updating Your Contact Information

It is your responsibility to provide Engage with an accurate and complete email address and other contact information. You can update contact information such as your email address and phone number via the website caremoney.us or the App. You may also update your email address and phone number by contacting us at 1 (855) 978-7011 or support@caremoney.us.
